This is a voluntary form that provides Security Services and first aid attendants with pertinent information regarding any medical conditions you have and/or any required treatments you may need. In the event of an emergency, this information can assist first aid attendants with administering life-saving treatment before emergency medical personnel arrive.
All information provided will be kept in the strictest of confidence in the ÂÜÀòÉäÇø Security Office and will only be shared with emergency medical personnel should the need arise.
